Summary: Polypharmacy has become a global health problem and is responsible for a variety of drug-related problems. For the WHO, unsafe medication practices and medication errors are leading causes of injury and avoidable harm in health care systems across the world. Better control of polypharmacy was named one of three key priority actions to improve drug safety and to reduce the level of severe, avoidable harm related to medications by 50 % over the next five years.Family doctors hold a key position in the medication process and in the management of polypharmacy. The article describes the individual steps of the medication review in the family practitioner’s practice and suggests general measures against medicalization and polypharmacy.
