Brief Primary Care Intervention for Adult Attention-Deficit Hyperactivity Disorder (ADHD) – a Feasibility Study

BackgroundAdult ADHD is common, unfortunately partially misdiagnosed and impairing, but sensitive for treatment. Beside medication, cognitive behavior therapy (CBT) has a positive impact on patient’s symptoms. How would a CBT-adopted intervention for family physicians (FP) look like? What about practicability and acceptability in primary healthcare? MethodsSix primary care practices (FPs 50 % female) were recruited for this prospective interventional feasibility study. After a structured training FPs included eligible patients (n = 11, ADHD diagnosis, 18–65 years) and performed the intervention (4 sessions à 30 minutes). Conner’s Adult ADHD Rating Scale (CAARS-S) was used pre/post for monitoring symptom severity. Finally, patients (via questionnaire) and GPs (via semi-structured interview) evaluated the intervention. Statistical frequencies were analyzed descriptively, exploratory. ResultsThe CAARS-S as well as the evaluation of patients and FPs showed a positive trend regarding indications of symptom severity and the patient’s state of health. FPs had a positive overall impression of the concept of four sessions. Time effort and the (still) missing accounting opportunities were criticized most. Psychoeducation and the mindfulness session were top rated. All patients would opt for the intervention again. ConclusionsThe investigated brief primary care intervention for Adult ADHD was rated helpful and feasibly from both sides, patient’s and FP’s. Subsequent studies have to examine, if the health care of adults with ADHD improves by integrating FP’s according a stepped care model.
