Family Physician-Centered Healthcare for Patients with Cardiovascular Conditions – Results from Germany

Background: Family physician-(FP-)centered healthcare (FPCH) is a program to strengthen the role of primary health care in the German healthcare system. Optimized primary health care is expected to improve treatment quality and outcomes in patients with chronic conditions. The aim of this study is to evaluate the effects of FPCH for patients with cardiovascular diseases.Methods: The present analysis is based on a retrospective cohort study on patients with cardiovascular diseases (coronary heart disease, chronic heart failure, atrial fibrillation or myocardial infarction) based on claims data analysis of a large health insurance fund in southern Germany (AOK Baden-Wurttemberg). We compare patients participating in the FPCH program to patients in regular care. A set of quality and outcome indicators is used. We report descriptive results and results of multivariate regression analysis that adjust for important confounding factors.Results: Patients with chronic cardiovascular conditions (coronary heart disease, chronic heart failure and atrial fibrillation) show improved outcomes in the FPCH program compared to regular care. Only for patients with acute myocardial infarction, no advantageous effects of the program became apparent.Conclusions: Results show that optimized primary health care through FPCH may improve quality and outcomes of care for patients with chronic cardiovascular conditions.
